Accreditation is a voluntary evaluation process for eye care services that organizations undergo to meet defined standards of quality agreed upon by members of an accrediting body. When an institution is accredited, it means that the eye care services have been evaluated and judged to meet the standards of the profession. Accreditation through the Ophthalmology Foundation focuses on evaluating and refining eye care services and ultimately improving patient care.

Accreditation Fees

When submitting the application, eye care services are required to pay a $150 USD application review fee (payable by credit card/bank account or wire tranfser).

After the accreditation committee determines the eye care service is eligible to proceed, a $12,000 USD accreditation fee is due (payable by wire transfer).

The eye care services will be required to pay a round-trip ticket in business class and two nights accommodation for a site reviewer.

Steps for Eye Care Services Accreditation

Eye care services accreditation is a process that requires standards of structure, process and achievement, self-assessment, and on-site review by outside experts. The process of eye care services accreditation typically comprises a four-step cycle.

  1. Complete Self Assessment: The program completes the Eye Care Services Accreditation Self-Assessment of their educational process, resources, strengths and weaknesses based on the Eye Care Services Accreditation Guidelines.
    • Contact info@ophthalmologyfoundation.org to request the self-assessment form.
    • Submit the self-assessment and supporting documents in the accreditation application form below.
    • Accreditation application review fee of $150 is to be submitted with the application, payable by credit card/bank account or wire transfer.
  2. Application Eligibility Review and Fees: The accreditation review panel determines eligibility to proceed with the accreditation process and provides recommendations to the eye care services.
    • If the eye care service wishes to proceed with evaluation of application for accreditation, an Accreditation fee is due of $12,000 USD payable by wire transfer.
  3. Self-Assessment Review and Site Visit: The accreditation evaluators will review the self-assessment and provide detailed feedback to the eye care services regarding deficiencies and corrective actions needed.
    • An in-person site visit will be arranged. The cost of business class airfare and two nights accommodation for one evaluator will be charged.
    • A program has six months from application eligibility review submission to schedule the site visit date or the process and payments will need to start over. 
  4. Eye Care Services Application Evaluation: an accrediting organization reviews the eye care service’s self-assessment and the site visit report to assure accreditation standards are met.
    • The evaluators make a recommendation based on the site visit and self-assessment.
    • The Accreditation committee reviews the eye care service’s self-assessment and the evaluator’s site visit report to assure accreditation standards are met.  
    • Full (5 years) accreditation or denial (with option to appeal) of accreditation of eye care service is issued as a certificate and plaque.
    • The accreditation process repeats at regular intervals of five (5) years.
    • A plaque acknowledging eye care services accreditation will be sent to the program.
